Preferred Plus, Preferred, Standard Plus, and Standard—the definitions and factors moving you up or down the scale.
Insurance underwriting uses categories to price policies. Each company names them differently, though the framework mirrors others': Preferred Plus (sometimes Elite or Super), Preferred, Standard Plus, Standard, plus higher-risk table ratings. Tobacco applicants face separate, pricier tiers.
What underwriters look at
Body composition (height, weight), vascular health (pressure, cholesterol), parental/sibling heart or tumor diagnoses prior to sixty, violation history, prescription patterns, occupation/recreation choices, and nicotine/tobacco consumption. Highest tiers demand strong marks almost everywhere; less favorable tiers accept limited, controlled conditions such as blood pressure management.

Rate yourself honestly on the form: Excellent lines up with Preferred Plus, Very good with Preferred, Good with Standard Plus, Average with Standard. Quotes reflect this level; underwriting confirms the ultimate level. Unsure which level fits? Request rates at two levels, compare, and allow underwriting to determine the real answer. Inflating your health rating doesn't reduce the actual premium; it just throws off your estimate.
